Fax cover sheets/cover pages are included by default when faxing with Kno2 Connected™ apps. The cover sheet automatically includes key details, such as the "from" fax number, to streamline communications.
Additionally, a metadata page is created from the patient information and comments added at the time of sending. It is placed after the cover sheet to protect confidential information on printed faxes.
Some Kno2 Connected™ apps elect to skip the default cover sheet in favor of one generated from within the EMR. If you are not sure how your cover sheet is generated, ask your EMR contact for details.

The default metadata page includes the following fields:
Date/time (added automatically)
From fax number (added automatically)
Patient ID (optional)
Patient Name (required)

Suppressing the Cover Sheet and/or Metadata Page
If your organization prefers no cover sheet or to manually add a cover sheet at the time of sending, or if you prefer not to send the metadata page, you may automatically suppress them following these steps:
Navigate to Settings (gear icon)
Select Organization
Optional: Check the box for Suppress Cover Sheet to disable to default cover sheet.
Optional: Check the box for Suppress Metadata Page to disable the default patient metadata page.
Note: This page is where comments are displayed; disabling it means no comments will be delivered to the recipient.
Click Save Changes.
Cover Sheet Customization
Some customization is available for the default cover sheet:
Organization or facility name (up to 80 characters)
Logo upload, automatically converted to black and white and sized for fax transmission, with a choice of small (1"), medium (1.5"), or large (2") printed width
Phone number (required, used in both the header and the confidentiality notice)
Up to four additional contact lines (address, fax number, website, etc.)
Cover Sheet title
Order of the Subject, To, From, and Date fields
Subject line: automatically filled in at send time, or fixed to a static value
From line: automatically filled in at send time, or fixed to a static value
Confidentiality/HIPAA notice text, fully editable, with a one-click restore to the default language
To customize your cover sheet, download the file Cover Sheet Builder - (Drag file to browser to use).html available at the bottom of this article and drag it onto your browser to open it. Your browser acts as an .html reader, working locally instead of sending anything over the internet. It lets you update your organization name and logo, add contact information, reorder the Subject, To, From, and Date fields, choose whether the Subject and From lines are filled in automatically or stay fixed, and edit the confidentiality notice text.
The Cover Sheet Builder will guide you through the customization process. When you are happy with the result, click the Download my cover sheet button to obtain a copy.
Email the cover sheet to kno2connected@kno2.com with the subject line "Fax Cover Sheet Update - [organization name as it appears in your app]." Please allow a minimum of 5 business days for the Kno2 Healthcare Network Services team to process your request.
If you do not use this exact subject line or include your organization name exactly as it appears in the app, processing will be delayed.
Once the cover sheet has been added for your organization, you will be alerted so that you can test by sending a fax to yourself.
Testing
Once the cover sheet has been added, login to the app and navigate to Release to send a fax to a number you can receive the fax at. This may be your own number, in which case the received fax will land in your EMR or in My Intake. Review the received sheet to verify your changes.
